(Savannah, GA) Two weeks after Giuliana Rancic shared her life story at the smart women luncheon in Savannah she was diagnosed with breast cancer.
Shortly after a double lumpectomy there's a new decision about her treatment.
Rancic with her husband Bill revealed the next step Monday morning on the Today Show.
"Now instead of radiation I'm going to move forward with a double mastectomy."
Reconstructive surgery will follow.
Lewis Cancer & Research Pavilion Medical Director Dr. Howard Zaren says he can understand Giuliana's thinking.
"The facts are they didn't get a negative margin so she's really still at risk, and she's at a higher risk than someone who had a negative margin.  So that's one of the reasons why she may have chosen this."
"With the double mastectomy I have less than a 1% chance of getting it back.  With another lumpectomy, radiation, and medication I could have seen 20, to 30, to 40% chance in my lifetime and for me it just wasn't worth it."
Rancic admits her dream to have a baby was taken into account, but her husband says the double mastectomy decision was not made lightly.
"One of the other factors that came into play was quality of life.  If she would have gone with the lumpectomy and the radiation then you have to go in every 6 months for the rest of your life getting a mammogram," says Bill Rancic.
The classy down to earth E! News Anchor is praised for doing a great service to women.
"She's a very brave woman in my opinion to share her private thoughts like this.  She has helped women tremendously in my opinion, and we're lucky we do know her and we do consider her a Savannahian whether she's a Savannahian or not she was here and she made a great impression on women of Savannah," says Dr. Zaren. Giuliana says her journey has made the love she and her husband share stronger.
"I couldn't be more at peace with the decision, but it was hard, and I still break down some nights you know when it's quiet in the bed.  It's easy to just start crying and be very sad, but I'm okay."  
Giuliana says there's no right or wrong decision when it comes to having a mastectomy. It's an individual decision.
